that time it was very clear that the pond in the rear of the
property had been enlarged and was cnverinn a rather large
part of the lot. Also at that time the previous instructions
as stated in the Decembpr 79 evaluation had not been followed.

and the proposed home will be set hack far enough to allow
propnr space for the installation of a sewage system. As
discussed with you on the site the following must be done
at once to insure the provisionally suitable classification
of lot # 27.(A.) Drainnoe wnys completed—tile -in place and
covered with earth. (R.) Snil in area of proposed installation

The following limitations and/or conditions shrill be in effect.
1. System shall be designed for no more than a three bedroom home.
2.. Drive must enter from the left hand side of property.
3. Hater line shall be installed down the side of the drive way.
4. This office shall design a system based on an application rate of
.4gpd.
5. It appears that a pump will be needed in order to get to the proper
area of installation. This will he determined when the permit is
issued.
It must be noted that should any of the items addressed in this letter not
be in compliance, the classification of lots 27 could be changed once again.

SUBJECT: Site evaluation lot # 27 Bermuda Run, Davie County
with Mr. Mando and Mr. Hall, Davie County Health Department
The above mentioned lot was visited and the following information is
provided for your information:
1. Lot # 27 has severe limitations for the installation of a functioning
ground absorption sewage treatment and disposal system. Limitations
include landscape position, drainage ways, surface drainage from the
adjoining property and the fluctuating water level of a pond on the
rear portion of the property.
2. A letter dated December 69 1979 addressed to Mrs Shirley Soderstrom
from the Davie County Health Department details the necessary improve—
ments which would be required for the property to be reclassified from
an unsuitable to a provisionally suitable state, for the installation
of a ground absorption sewage treatment and disposal system.
3. The improvements recommended in December 6, 1979 letter, relative to
lot # 27 would need to be made and the exact location of a proposed
home be staked out before further evaluation of the site would be
possible.
4. It is imperative that those parties interested in this lot and the
property owner fully understand that the development of this lot as a
home site will depend upon drainage problems being eliminated which
would certainly contribute to a premature failure of any ground absorption
sewage treatment and disposal system installed.
5. Perhaps one alternative would be to provide public sewer for lot # 279
however, the existing drainage problems could lead to structural failure.

Dear Mrs. Soderstrom:
On December 4 and 5, 1979, this office evaluated the above mentioned
property`. Also present at the evaluation was Mr. James Ress, Testing
Labarotory, Inc. from Greensboro, N.C. The soil conditions -and percolation
rate,�are such that we can classify this lot as provisionally suitable.for
a conventional
ground absorption, sewage disposal system to be installed.
As -.1 pointed out to you earlier it is imperative that my office meet with
the future homeowner and/or builder in order to designate the only area;
available for a sewage system to be installed.
This office will issue an improvements permit(for the installation of
a conventional ground absorption seage disposal system),ufider the following
conditions and only if all conditions are met.

1. Drainage way from road is diverted off of property :and
drainage'System be implemented to eliminate surface water
from .adjoining lots.
-2: That the entire area evaluated and classified as provisionally '
suitable(facing lot from road -would be upper most left front
portion of lot) be staked off and all necessary trees be eliminated
there shall be no driveway drainways, etc. in this area.
3,The size of the system shall be based on..,A.three(3) bedroom,
home (due to lack of suitable space). An application rate
of 0.4 gpd/per square foot shall app*. This will caU:- for a .
minimum of 375 linear feet of nitrification line. The system',
shall be no deeper than 30":below, present grade, thus allowing
full use.of the better layers of soil. Due to;topography and
possible'location of the house, it`may be necessary to install
a lift station in order to pump the effluent to the desired
location of the sewage system.

At present I am the Environmental Health Coordinator at the
Davie County Health Department. I am a registered sanitarian and
have been for the past 3 years. I have been employed as a sanitarian
for the past 10 years.
Subject: Lot #27, Bermuda Run
Davie County
The_ aforementioned lot was evaluated by representatives of the
Davie County Health Department, Environmental Health Section, on
several different dates, the last being November 2, 1982. The results
of all evaluations reveal the same findings, that being a soil con-
dition which would allow for the installation of a ground absorption
sewage treatment and disposal system. Furthermore, due to the avail-
able space to install a system, the site was classified provisionally
suitable. Due to the different levels of the small lake adjoining
the property the location of the dwelling can not be determined.
Should the dwelling have to be moved further up the property, the
site would then be reclassified as unsuitable, due to lack of space
for the installation of the sewage treatment and disposal system.
