The undersigned consents to and hereby gives notice of
the following:
l. The undersiged is the owner of that sixteen (16>
acre tract of property described in Deed Book 119, Page 583.
2. The Davie County Health Department issued a permit
to install a septic tank/drainfield, and such permit was
predicated on the entire sixteen (16) acre tract being
available for septic tank and sewer line purposes.
3. Subsequently, the septic tank and other
improvements were built on the following described property,
which is only a portion of the property considered for the
original septic permit:
The BEGINNING point is located by going North 26 deg. 59'
22" West 691.80 feet from the N.C.G.S. Monument identified
as "Woodward" (N = 802,799.770, E= 1,541,326.351) to an
iron pin on the north right of way line of Woodward Road
(State Road 1407); from said beginning point which is a new
corner for the Yokley land (Deed Book 119, Page 583) going
thence North along the northern right of way of Woodward
Road 37 deg. 29' 32" West 152.13 feet to a point and North
39 deg. 13' 21" West 156.39 feet to a new iron; then�ce on
the following new courses and distances: North 52 deg. 29'
23" East 308.60 feet to a new iron; South 15' 13" 40" East
314.30 feet a new iron; thence South 47 deg. 02' 37" West
185.63 feet to the BEGINNING point; being 1.677 acres, as
surveyed and platted by Grady L. Tutterow, R.L.S., and
identified thereon as Drawing No. 16484-3 dated January 31,
1985. Being a portion of the property described in Deed
Book 119, Page 583.
4. The Davie County Health Department has threatened
to withdraw the permit unless all of the subject land is
available for alternate septic tank and sewer treatment
facilities and such dedication is provided in writing.
5. It is, therefore, the intent and act of the
undersigned to dedicate the balance of the property
described in Book 119, Page 583, for use as an alternate
septic tank field to service the above 1.677 acre tract
under the plans and requirements as may be mandated by the

Davie County Health Department.
